Influenza Vaccine (Age 65+)
- Requirement: Patients must have received the flu shot during the current season (up to January 31, 2026).

- Max Bonus: $2,200 (at 80% compliance).
Childhood Immunizations (Aged 30–42 Months)
- Requirement: Children must have completed their full primary series by their 30th month.
- Tracking Code: Q132A.
- Max Bonus: $2,200 (at 95% compliance).
Cervical Cancer Screening (Ages 25–69)
- Requirement: One screening within the 66 months prior to March 31, 2026.
- Tracking Code: Q011A.
- March 2026 Update: As of March 3, 2026, the laboratory service L698 – Human Papillomavirus (HPV) Screen has been officially added to the list of reported services. This ensures you get credit for modern HPV-based screening protocols.
- Max Bonus: $2,200 (at 80% compliance).
Mammography (Ages 50–74)
- Requirement: One screening in the 30 months prior to March 31, 2026.
- Tracking Code: Q131A.
- Max Bonus: $2,200 (at 75% compliance).
Colorectal Cancer Screening (Ages 50–74)
- Requirement: A FIT test every 2 years or a Colonoscopy within the last 10 years.
- Tracking Code: Q133A.
- Max Bonus: $4,000 (at 70% compliance).
3. Critical Deadlines You Cannot Ignore
The window for these bonuses is narrow and strictly enforced by the Ministry of Health.
- March 31, 2026 (Snapshot Date): All screenings must be performed, and all tracking/exclusion codes must be entered into your EMR with a service date of March 31 or earlier.
- June 18, 2026 (Submission Deadline): This is the final date to submit your bonus claims. Any submission received after this date is considered “stale-dated” and will be automatically rejected.
4. How to Maximize Your Payout: The Exclusion Strategy
The secret to hitting Level 5 payouts isn’t just seeing more patients—it’s excluding the right ones.
By correctly billing exclusion codes (at $0), you reduce the denominator and instantly boost your score. Key codes include:
- Q140A: Total Hysterectomy (Excludes from Pap category).

5. Why FHO Physicians Are No Longer on This List
As a reminder for those in transition: 2024 was the final year FHO and FHN physicians could claim the three cancer screening bonuses. If you are in a FHO/FHN, your cancer screening incentives have been replaced by the Acuity Modifier, which is now a permanent monthly automated payment added to your capitation. FHO/FHN docs only remain eligible for the Influenza and Childhood Immunization categories.
